Homework assignment to client/group

1. Balancing Exercise: I provided a sheet with images and instructions for balance exercises, reminded him to
practice safely with a clear surrounding, and use a table or wall for support.

2. Lower body muscles strengthen exercises

3. Eye movement: 1 or 2 times/day and as needed.

4. JAPA Meditation with Alternate Nostril Breathing, suggested combining his 10-minute daily practice
with the affirmation, “I trust my body’s ability to heal and restore itself,” which became his favorite mantra.

Activities

The session began with a check-in where Dan mentioned no major changes since the last session, which was 2 weeks ago, as he was traveling to see his family. He shared that he is practicing the "chin-down-plus-larynx-tightening maneuver," which he found helpful and planned to continue as he understood that achieving natural swallowing would take months. He said liquid is easier but sold foods are more challenging.

To start our session, I led Dan through a centering exercise using three-part breathing which became one of his daily routines then eye movement another routine. He shared his experience at the airport last week, he sat for 5 minutes for breathing practice to relax his mind as he described that when he got out of the car it was extremely stressful at the airport.

For physical exercises, I led Dan through joint movements and slow shoulder and back movements, emphasizing the importance of deep breathing with each motion. We then focused on balancing exercises for about 10 minutes.

To conclude the session, I led Dan through a 15-minute session of Yoga Nidra.

Client/Group progress summary

Dan’s breathing practice, eye movement, and affirmation are helping him a lot. And he can apply those tools when he needs them. For philosophical parts of practice, he is also learning a lot. He described affirmation makes him calmer, and he is less anxious and is more patient now.

Reflection and self-evaluation

When I started my practicum with Dan, I had questions to myself, about how much I could help him physically and mentally. He went through a very challenging life past 2 years and still dealing with physical limitations. As I hear his experience or see his improvement every week, I see the plan is working and am more comfortable working with him.

Plan for next session

I will continue to follow this care plan for the next several sessions, I would like to focus his physical practice. He needs to continue practicing to improve his balance.
