Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Activities

Check in and Discussion: Our consciousness is elevated by devoted practice (Abhyasa). by nurturing ourselves and our practice our hearts and minds become dedicated and we develop a steady rhythm and flow into our daily lives.
Centering- body scan, gathering information on what we need out of our practice.
Asana- Japa, Salutaions, Restorative twists, side bends, back bends and folds. inversion legs up a chair.

pranayama- sare breath with imagery of a circle surrounding her with calming color and creates a safe serene space
Meditation- absorbing the benefits of the moment and breath and listening to the transformations, that it is here we can carry our practice off the mat and into our lives, that it is here we are nurturing ourselves.

Client/Group progress summary

She is enjoying the teachings and breathing, still working on her presence and stillness. Which is why we incubated restorative poses for her today. To settle on to the support feeling her body relax and release.

Reflection and self-evaluation

This client wanted to move a bit which is why we did the salutations, but she is having another surgery and I wanted her to be able to use the calmer practice during her healing, to remind her of her breath and gently loosen her joints.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

We will be on a short hiatus, so will see where she is after her healing, which is why we have 90 minute sessions because our checking in time is crucial for her plan.
