| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |
| Number of sessions completed | 6 |
| Date you started seeing client/group | 04/11/2024 |
| Total hours of all Yoga Therapy sessions with this client/group to date | 6.25 |
| Adjustments and adaptations you made to your care plan, | The client found moving meditation with hand gestures irritating-too much thinking involved. We removed this technique from their care plan. On several occasions, the client showed me their go to stretches and we either practiced them together, or I offered a variation or two, then we tried the variation out together inquiring along the way. |
| Client/Group Goals | Together, the client and I have created an adaptive yoga practice, combining some of their established stretching movements with some new movements I have introduced and pranayama focusing on the exhale and a variety of guided relaxations. They feel less discomfort while stretching and are able to sustain a stretch for longer than when we first started working together. |
| Report briefly on each Kosha below | Progress toward wellness or worsening reported by the client/group or that you observed in the following areas |
| Physical level | Slightly more acceptance of chronic pain, finding relief with mindfulness and guided relaxation. Dancing brings the client joy. They make sure to stretch a good bit prior to dancing. They are ready to attend to subsequent pain and fatigue after dancing which is less if they stretch before. |
| Energetic level | The client has found a short guided relaxation or brief meditation practice mid-day supplements their typically restless sleep and boosts their energy level for the remainder of the workday. |
| Emotional | The client’s level of anger has slightly diminished. Their friendships help to discharge the anger through conversation and sharing. Some anger is also discharged with mindful movement, stretching and breathwork. |
| Intellectual / Sense of self | The client readily experiences themselves as spirit, easily accessing the subtle dimensions of being. This is their happy place. |
| Spiritual orientation and support plan | This is a source of strength for the client. They meet regularly with a shamanic group for shared practice and community building. |
| Additional Information | |
| Feedback received from client/group, anecdotal or written | The client looks forward to our sessions. They consistently mention feeling refreshed and calmer after each session. |
| Sample of homework given between sessions (after initial homework) | Centering, grounding. Seated cow/cat, hip circles, Sufi circles, standing joint freeing movements for the lower body, SMTT mini-set, standing asana to relieve stress, quad stretch, seated pigeon, extended exhale breath practice, cosmic orbit meditation with So Hum mantra repetition. |
| Personal reflection from doing client/group. | |
| Rough estimate of time spent in preparation and follow up documentation per session | 30-45 minutes to prepare, 30 minutes to document. |
| What you would change with benefit of hindsight | Inquire into the client’s pain perception/experience more fully. I tend to shy away when a client says they sense pain, retreating. Making a change to alleviate the pain and taking some time to assess the pain experience may serve better serve the client. |
| Questions, problems, areas in which you’d like more support | None at this time. |
| Did you enjoy your service? | Yes, very much so. I believe the client has moved from skepticism of me to trusting in me. |
