Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Standing wall lunges, shoulder rolls, hands in and out breathing

In bed, have client bend knee, place foot by ankle (modified figure 4) pillow under knee - help gently open hip

Arms up and down breathing

Arms open and close breathing with extended exhales when we opened our arms out

Wisdom: Interception & proprioception: how his body felt while breathing, eyes closed and moving arms. During rest how his mind, breath, body felt after those movements. Weaved it all though out class

Warm up:
shoulder rolls
Neck stretch

Seated: With strap, Arms over head arms in Y shape, then bend at the elbows to bring arms behind head to a W shape - Unable to bring arms behind him in W & unable to bend arms in W. Just worked on pulling strap apart in Y shape
Chest press 7x 3 sets with light resistance band
Chest pulls 7x 3 sets
strap under foot, leg raises & extensions - hips very tight
light resistance band under foot, arm raises 7x left arm was not able to raise as high at first but by last set able to raise much higher

Press down on foam block with foot 3x 10,15,20 sec

Seated cobra
Shoulder rolls
Wrist rolls

Posture strap
Put Legs on chair to help them stretch and help with flexibility by allowing them to slowly release during shavasana

Yoga nidra - body scan meditation - client said he was very very relaxed

Client/Group progress summary

Frank just got back from vacation. He said he was feeling lazy because of the hot weather and vacation but was happy to be doing YT. He said without YT he wouldn't have done so well on vacation with all the stairs and walking in Italy. He said he really felt the difference. 🙂

Clients hips were tight. unable to bend elbows over head, but progress that he is now able to raise his arms overhead

Reflection and self-evaluation

Easing FA back into YT after vacation. Enjoyed him sharing and listening to his vacation stories. He really enjoyed himself with his wife.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
