Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Activities

The client has arthritis of the right hip with osteoporosis of the femoral neck. I continue to work with her on improving mobility, increasing strength and managing pain through asana, pranayama, meditation and yogic philosophy.

• Checking in and assessment: PROMIS Global, PEG scale and Arthritis Self efficacy. Philosophy – practice and non-attachment and ahimsa
• Centering: body scan and breath awareness
• Joint movement: cat cow, side bend, shoulder shrug, internal and external rotation, knees and ankles
• Hip movement with chair: standing and circle the leg, lateral lift, and bring the leg forward and back (with breath)

Client/Group progress summary

Both PROMIS Global Health and PEG assessment indicated improvement with PEG score improved by over 1 point. Arthritis Self Efficacy Scale shows the client feels moderately certain to certain (average 75/100, range 60-80 with 100 being very certain) in her ability to continue her daily activity and use methods other than taking extra medication in small to moderately reduce her arthritis pain. When asked what she thought was the most helpful, she stated that reframing and shifting her thought about pain is very helpful. She also finds breathing practice and asana to help reduce the pain.

Reflection and self-evaluation

The client struggled with subtle practice. In the last session, we discussed about the benefits of meditation and breathing in pain management. Since then, she meditates 5-6 minutes daily and is more consistent with breathing practice. She also is motivated to do more. So, explaining 'the why' is important.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

Continue to work on improving mobility and strength and coping with hip pain. Activities will include grounding asanas (standing poses, forwarded bend), deep relaxation, pranayama (3-part breathing, alternate nostril, extended exhalation) and meditation.
