  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)

with the affirmation, “I trust my body’s ability to heal and restore itself,” which became his favorite mantra.

Activities

I began the session with a check-in where Dan mentioned no major changes since the last session, which was 2 weeks ago, because of the holiday week. Dan reported that he hadn't been able to physically practice much in the past weeks due to a persistent cough which he believes is a summer cold. But he has been practicing meditation, breathing, and eye movement daily.

Then I led the guided meditation to bring him to the present moment. Next, we moved to physical practice. We started with slow shoulder and back movements, emphasizing the importance of deep breathing with each motion followed by joint movements, today he agreed to practice on the mat instead of the chair. He shared that he felt some of the joint movements, like hip rotations, were more intense on the mat compared to the chair, which he liked. After this, we did balance practices and muscle stretching exercises. He agreed that we focus on his physical practices to improve his balance and strength.

To end the session, I led Dan through a 15-minute body scan for his physical and mental relaxation.

Client/Group progress summary

Dan shared that his mindset and emotional state became positive. He said he turned 64 yesterday, and despite his concern about financial security, he felt more at ease and capable of managing his thoughts and emotions. He also said he shared this thought with his wife Annie.
He reported that he meditates for 60 minutes on Monday, Wednesday, and Friday. I suggested him to meditate every day. This is a significant progress since he didn't meditate at all two months ago. He enjoys eye movements, breathing practices, and repeating his favorite affirmation.

Reflection and self-evaluation

I felt good to see him more positive than two months ago when he wrote about his frustration with the company he used to work for and his concerns about his financial future. Also I am glad he remembers that I told him “Nothing lasts forever, everything is temporary”.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

I will continue to follow this care plan for the next several sessions. I will reassess his physical movement and focus on balance and strength, as these are his concerns as well.
