Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Homework assignment to client/group

Asked client to keep trying to take deep breaths when she is stressed and though out the day. Continue to shake her arms and hands when she feels overwhelmed. Continue with daily cat/cow and child’s pose, watch her posture, seated and standing through out the day.

Activities

Grounding, centering, extended exhale
warm up- on back with somatic movements. slowly opening hips, activating pelvic floor, moving arms - all with breath

Table top: continued with somatic movements-cat/cow, Childs, cobra
Gate pose- open hips

Bird/dog - slowly building to bird/dog crunches, added: bear pose

Worked the spine in all directions.

Continue to strengthen and lengthen. Continue with Sun A, anjaneyasana.

Moved slowly today with longer holds. Did back bends today with salabhasana pose. Started slowly with just legs, then just arms, then full pose

Cool down, relaxation and meditation: We ended with a kindness meditation for just herself. Just picturing herself and meta meditation just for her.

Asked her how she did with giving herself grace and kindness. She said she tried but gets caught up in the moment. She knows she should pause and breathe but it's difficult for her to do when her kids are crying and tugging on her.

Client/Group progress summary

Continued with strengthening and lengthening muscles/flexibility. Client said she wished she can come to me more than 1x a week. She can feel her low back start to relax from the pain and she feels so calm when she leaves.

She said her kids loved Childs pose and cat/cow and now they do it together-great bonding time with her kids. She found a Children yoga tape that they do.

Reflection and self-evaluation

Session went well. Client was very receptive. Will continue to strengthen her back by adding some more backbends and lengthening with forward bends.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

Continue to lengthen and strengthen her back and with all movements of the spine. Continue with more backbends. Continue with breath work of deep breathing and go keep giving herself grace. Will show her some tapping and adding in the mantra of "I'm doing the best that I can" while tapping.
