Tools from each module should be used (not on each client – but overall) |
| Session Date | 07/14/2024 |
| Session Number | 5 |
| Total Session Minutes | 60 |
| Homework assignment to client/group | The surgeon has instructed the client to walk 10 minutes every hour. Therefore, I recommended the client to incorporate walking meditation that focus on the breath and motion of his arms and feet for a few minutes once or twice a day. |
| Activities | The client recently had a laminectomy for lumbar stenosis: 12 days post-surgery. No physical therapy until 6 weeks after surgery. Although he can lie down on his back, but the surgery site is still sore and tender. • Centering – body scan, breath awareness (extended exhalation) |
| Client/Group progress summary | The client is recovering from the surgery. There is no pain post-surgery, and he is able to walk. He can lie on his back and side, but the surgery site is still tender. |
| Reflection and self-evaluation | The class was very gentle. The client was able to go down to the floor for restorative poses very easily and was comfortable during the poses. However, he needed assistance getting up from the floor. I feel that laying down on a bed or sitting on a chair would be better than the floor. The client experienced tingling sensation on his right foot after walking meditation. I think that it is because he tensed the body (i.e., back) to keep balance when walking at a slow pace. I suggested that instead of walking very slow, he can walk at a normal pace, but be mindful of his breath and how he feels while walking. |
| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |
| Plan for next session | The client will see his doctor on July 24th. I will check with him after his doctor visit and will plan the next session accordingly. I will continue to avoid rounding and twisting of the spine. Asana practice will be done on a chair. I will also introduce bee breath in the next session. |
