| Session Date | 07/18/2024 |
| Session Number | 9 |
| Total Session Minutes | 75 |
| Homework assignment to client/group | Homework assignment to client/group 1. Balancing Exercise 2. Lower body muscles strengthen exercises 3. Eye movement: 1 or 2 times a day and as needed. 4. JAPA Meditation with Alternate Nostril Breathing, suggested combining his 10-minute daily practice with the affirmation, “I trust my body’s ability to heal and restore itself,” which became his favorite mantra. |
| Activities | I began the session with a check-in where Dan mentioned no major changes since the last session. He reported no change since the last session which was 1 week ago. I led a guided meditation to help him relax his breath and mind. Next, we moved to physical practice, starting with neck movements and slow shoulder and back movements, followed by joint movements. Then, he enjoyed balance practices and muscle stretching exercises. Instead of the tree pose, he lifted his knee up to a 90-degree angle and held the pose. He was comfortable with hip flexion and extension, but hip abduction was challenging for him. He agreed to practice these daily. |
| Client/Group progress summary | I noticed the improvement in his balance, especially right side. He can now stay much longer without touching the chair when he maintains standing on one leg. |
| Reflection and self-evaluation | He is noticing the improvement in his balance and has a positive attitude to physical movement practice because of the result. Some of the movements are still not easy but I am confident he can continue to improve. |
| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |
| Plan for next session | I will continue to follow this care plan for the next several sessions. I will assess his foot again and pay attention to his joint movement, especially on his foot before the nerve mapping procedure. |
