Tools from each module should be used (not on each client – but overall) |
| Session Date | 07/25/2024 |
| Session Number | 10 |
| Total Session Minutes | 75 |
| Homework assignment to client/group | 1. Balancing Exercise (one leg standing, heel-to-toe walking) 2. Lower body muscles strengthen exercises 3. Eye movement: 1 or 2 times a day and as needed. 4. JAPA Meditation with Alternate Nostril Breathing, suggested combining his 10-minute daily practice with the affirmation, “I trust my body’s ability to heal and restore itself,” which became his favorite mantra. |
| Activities | I began the session with a check-in where Dan mentioned no major changes since the last session. He reported that there have been no changes since the last session. His outdoor activities are limited to the weather and wildfires in Canada. I began the session with 3-parts breathing practice today. Next, we moved to physical practice, starting with neck movements and slow shoulder and back movements, followed by joint movements. He now uses a strap for some stretches like cow face for shoulder stretch which is working well. For final relaxation, I offered a 15-minute yoga nidra to relax physically and mentally. |
| Client/Group progress summary | He said the biggest improvement is that he can better control his mind. In addition to his medical condition, he has a work-related lawsuit in progress; even in these circumstances, he feels less anxiety and stressful. |
| Reflection and self-evaluation | His improvement in emotional state is helping to improve his energy and physical level as well. It feels so good to see his improvement at each session. From the beginning, he has shown no hesitation in any of these practices. This fact and his willingness are the factors in this improvement. |
| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |
| Plan for next session | I will continue to follow this care plan for the next several sessions. I may add new physical movements based on his progress. |
| Report briefly on each Kosha below | Progress toward wellness or worsening reported by the client/group or that you observed in the following areas |
| Additional Information | |
| Personal reflection from doing client/group. | |
| Notify Mentor? | Do not notify Mentor (choose if you wish to continue working on this entry later) |
