Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

1. Alternate nostril and leg up the chair daily.
2. Mindful of energy level - plan daily activity based on how much energy it takes to complete a task and how much energy she has on a given day (spoon theory).

The client has been diagnosed with stage 4 cancer; she also has chronic inflammation.

Client/Group progress summary

The client feels better from the last session. She has more energy and can take short walks, although she still does not get enough sleep (feeling dehydrated and needs to get up to drink water and goes to the bathroom).

Reflection and self-evaluation

The client could not sit comfortably on a chair, so I modified the plan to only include mostly supine and standing poses and keep seated poses to minimum. The client seemed to have a lot of energy (hyper) at the beginning of the session. The goal was to offer grounding practice to address Vata imbalance. At the end of the session, she was calmer (talked slower and more focus); it was challenging to discuss homework or wisdom teaching at the beginning of the class. I think that chanting is helpful (keep her center). We discussed Bramacharya. I feel that while she understands the concept, she easily gets carried away and is not mindful of her energy level.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

Continue with grounding practices. Depending on her energy level, the session may start with gentle movement and standing asanas followed by restorative poses and guided imagery. Box breathing or Bhramari will be introduced in the next session.
