| Session Date | 07/24/2024 |
| Session Number | 2 |
| Total Session Minutes | 75 |
| Homework assignment to client/group | 1. Balancing Exercise (one leg standing with knee 90 degrees angle) 2. Heel-to-toe walking 3. Alternate nostril breathing 4. JAPA Meditation with Alternate Nostril Breathing, suggested combining his 10-minute daily practice with the affirmation, “I am not limited by my body’s condition”. |
| Activities | This was the 1st session after intake/assessment. I explained an overview of the plan and benefits of each practice. He understood the concept of “I have an injured hand, I am not an injured hand” and said, “I will try”. I led a few minutes of 3-parts breathing to relax his body and the breath. Then brief body scan practice, and neck/ shoulder movements followed by joint movements. I skipped the neck extension which he feels pain when he looks up. I suggested to extend elbows when he moves his arms up and paying attention to his posture. I offered a Then we moved to balancing practice. In addition to one-leg standing and heel-to-toe walking practice, he could walk slowly, but I asked him to have someone support him in this practice. I offered a yoga nidra sitting on the chair to relax, demonstrated alternate nostril breathing, and practiced together. |
| Client/Group progress summary | Since this is the 1st session, I did not see any improvement. I expect gradual improvement he practices daily |
| Reflection and self-evaluation | This is a very challenging case. Mack is experiencing pain due to spinal issues and has limited movement, respiratory problems prevent him from comfortably performing daily activities. He is motivated to start a regular physical movement, and I expect to see his progress gradually. The primary goal is to improve his balance and reduce his pain. I believe the current plan can help achieve these goals |
| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |
| Plan for next session | I will continue to follow this care plan for the next several sessions. I will include how to move to/from the floor from/to the chair, and heel drop practice if he is comfortable. |
