Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Grounding, centering, breath work-buzzing bee

Warm up

Strengthening and lengthening movements to help with back: Used a small pilates ball. squeezed ball between hands-over head side bends. Ball between knees-bridge, ball against wall-gentle shoulder circles
bird dog to bird dog crunches. Plank pose to forearm plank to dolphin.
Sun A - high lunge, forward fold, twists . Each pose, holding for 7 breaths with muscle engagement. Building stamina, strength and concentration.

Client has also been taking my restorative class during the week.

Client said he went hiking recently and really enjoyed connecting back to nature. We spoke about Ayurveda - the sister science of yoga and how it uses the 5 five elements earth, water, fire, air, and ether to make up our dosas and each element is associated with a chakra. I told him to look up Banyan products website, use the free test to see what his doshas are. I suggested maybe his earth element / root chakra is out of balance because he feels unstable, loss sense of security as he recently lost his job and that's why connecting back to earth in hiking is exactly what he needs.

Client/Group progress summary

Client's low back has stabilized. He is feeling less pain or no pain. He is enjoying the extended yoga nidra at the end our session and is taking a restorative class during the week.

Reflection and self-evaluation

I was so shocked the dosas, chakra's and Ayurveda came to mind when he spoke about nature. I used the tool box of yoga to help connect the client with his chakra's and understanding that his root chakra and earth element might be unbalanced because he recently lost his job.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

Follow up with client on his dosha finding. Continue with strengthening and lengthening the muscles of his low back. Adding more balancing poses to help client feel grounded and stable.
