| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |

| Total hours of all Yoga Therapy sessions with this client/group to date | 420 |
| Adjustments and adaptations you made to your care plan, | I spent more time on breathing practice and yoga philosophy teaching than on asana practice because I believed it was more important for improving her mental well-being. She comes to the studio for asana and restorative yoga practice. |
| Client/Group Goals | Katie's goal is to achieve a holistic balance within her mind, body, and soul. She aims to alleviate tension in her back, neck, and shoulders while learning effective techniques to manage stress and anxiety. Improving sleep quality is also a key focus for her. Her top priority is managing stress and anxiety. |
| Report briefly on each Kosha below | Progress toward wellness or worsening reported by the client/group or that you observed in the following areas |
| Physical level | Katie reported that she is more mindful of her physical movements, making an effort not to hyperextend and to move slowly. She has observed slow progress in areas that have been causing a lot of pain, particularly in her back, neck, and shoulders. I agree with her observation that she moves more slowly than before. I suggested practicing some of the asanas with her eyes closed, which is helping her to enhance her mindful movements. |
| Energetic level | She reported that “I've seen moderate improvement in my energy but overall feeling more positive energy within me daily.” I noticed her breathing is deeper now than in our 1st session. |
| Emotional | I observed she became calmer as she doe morning mindful routine before our session. |
| Intellectual / Sense of self | Katie said she felt like she knew herself better than before, and she was filled with self-love and acceptance. She brought different feelings to each session. I provided a summary of teachings via email and discussed them at the following session. I started talking about 'yoga citta vritti nirodhah' in the first session, and based on her feelings at the session, I explained the four locks and keys, as well as ahimsa. She reads, understands, reflects on these concepts, and tries to apply them in her daily life. |
| Spiritual orientation and support plan | She seeks to enhance her spiritual well-being through deeper understanding and practice of yoga, meditation, and energy healing technique |
| Additional Information | |
| Feedback received from client/group, anecdotal or written | She provided feedback listed below, |
| Sample of homework given between sessions (after initial homework) | Below is the sample of homework assignments to Kaite, I provided Sadhana Chat or the use of a timer to remind her of a 2-times/day routine during work hours to reduce physical and mental stress. 1 Slow-paced Sun Salutation after morning exercise as moving meditation |
| Personal reflection from doing client/group. | |
| Rough estimate of time spent in preparation and follow up documentation per session | 75 |
| What you would change with benefit of hindsight | I think it will use a similar care plan. I mostly used yoga nidra for final relaxation, I could have used visualization / witness practice to improve her physical neck pain, and remove her emotional stress. |
| Questions, problems, areas in which you’d like more support | Katie is also seeing a talk therapist, so she tends to talk a lot. |
| Did you enjoy your service? | I did enjoy the sessions with Kaite, she has been practicing yoga not a long time. She is interested in different areas in yoga practice, philosophy, asana, ayurveda, chakra, and more. She enjoys listening to those topics, and I liked teaching her. |
