Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)

Homework assignment to client/group

Below are the home practice assignments, the progress will be assessed, and new practice may be added accordingly. I added 4 seated yoga poses for her digestive problem and neck movement practice with visualization

• gentle shoulder rolls and movements
• supine butterfly, supine spinal twist, knee to chest pose, child pose
• neck movement practice using visualization
• daily inspirational reading and reflection
• alternate nostril breathing / brahmari breathing

Activities

I began the session by asking if there were any changes physically and mentally. She reported no major changes physically since we met last time.
Also, she said she started experiencing the benefits of daily reflection and writing practice after reading and breathing practice.

I led a few minutes of alternate nostril breathing to relax her mind. As I sent
how to practice this breathing, she practiced on her own and she looked comfortable with this breathing.

Next, we moved to physical movements. I skipped neck movement as she expressed discomfort on neck movement at the previous session, and I led shoulder movement practice. This is a movement flow I use for all my clients, and it is a slow movement like tachi. As she addressed her digestive problem and mental stress caused by this, I added seated asanas to her daily practice. I demonstrated those asanas and practiced with her.

To end the session, I invited her to 15 minutes of yoga nidra practice to relax to physical body and calm her mind.

Client/Group progress summary

She is getting the benefits of daily reading/writing, reflection on reading reminded her of the teaching of acceptance in the bible.
Also she noticed the breathing practice is helping her to reduce mental stress.

Reflection and self-evaluation

This care plan is working well, especially in regulating her emotions using the techniques of reading, reflecting, and writing. And by adding breathing practice she can manage physical symptoms better. She enjoys gentle shoulder movement, I sent her the video of this movement so that she can practice on her own.

Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn.
