| Total Session Minutes | 60 |
| Homework assignment to client/group | Her osteopenia may lead to osteoporosis I added heel drop exercise • Alternate nostril breathing |
| Activities | I began the session with a check-in asking if there were any changes physical or mental. She reported no major changes, she takes pain medication for her LBP on average once a day which is less than before. Also, she addressed the daily care of her mother and handicapped son keep her busy. In addition, her husband had a health issue over the weekend, so she did not have time for any practice. We discussed how to make practice as a routine. I explained the importance of self-care, self-compassion, non-harming yourself, and the importance of daily practice even busy time taking care of others. I led 3 minutes of 3-parts breathing to bring her to relax the body/mind to prepare for the practice. For physical practice, we began with neck/shoulder movement and practiced seated cat/cow, spinal twist and knee to chest pose, and heel drop exercise. She said she can practice those in the bedroom which is carpeted floor. We ended the session with 15 minutes of yoga nidra to relax the body and calm the mind. She agreed to reflect and write after daily reading with her mother, I gave a few options how to this practice easier, writing on paper, using an iPad or voice recording etc. |
| Client/Group progress summary | She is comfortable practicing alternate nostril breathing, she can use her fingers smoothly. |
| Reflection and self-evaluation | She reported she has been taking pain medication once a day recently for her LBP, which is less than before. I think this improvement is due to her 60lbs loss of weight. I suggested using breathing to manage pain. I will offer brahmari breath at the next session. |
| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |
| Plan for next session | I will follow this care plan, I pay attention to her physical practice as she hasn’t done exercises for a long time. |
