Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Homework assignment to client/group

1. Balancing Exercise (one leg standing, heel to toe walking, hip abduction)

2. Lower body muscles strengthen exercises

3. Eye movement: 1 or 2 times a day and as needed.

4. JAPA Meditation with Alternate Nostril Breathing, suggested combining his 10-minute daily practice with the affirmation, “I trust my body’s ability to heal and restore itself,” which became his favorite mantra.

Activities

I began the session with a check-in where Dan mentioned no major changes since the last session. He reported that there have been no major changes since the last session. Also, he reported he started heel-to-toe walking with his wife Annie, and early morning bike ride when the weather permits.

I began the session with 3-parts breathing practice today. Then, we moved to physical practice, neck/shoulder stretch and JAPA on the floor. I noticed he was more comfortable with JAPA using a strap. Then we moved to standing practice, one leg standing, hip flexion, hip extension, half locust pose holding chair. Also, he began practicing tree pose, heel above, and ankle to start.

He was able to relax both physically and mentally after 15 minutes of yoga nidra

He continues to show improvement at every session especially his balance is being improved I observed it past 2 -3 sessions

Reflection and self-evaluation

He agrees with his recent physical improvement, and I think this awareness of improvement has become his motivation to practice.
I believe introducing new practices gradually is helping to keep his motivation for daily routines

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. Plan for next session

I will continue to follow this care plan for the next several sessions, especially focusing on physical strength to improve his balance.
