| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |

| Adjustments and adaptations you made to your care plan, | Incorporated compassionate access to the pelvis, using the svadisthana mudra and bringing loving kindness to this part of her body. Also introduced flowing movement with practices such as a moving meditation to connect to the water element, the element of the svadisthana chakra, and create an upward flow of energy. |
| Client/Group Goals | Practices to lift mood and invigorate have shown positive results for the client, often generating laughter and humor. Stretching and core strengthening has begun to alleviate some of the client’s LBP. The client shared the inaccessibility of her pelvic region since her emergency C-section and following death of her baby girl. She subsequently shared this with her husband. We included awareness and somatic accessibility in this area as part of her care plan. The client realized the connection between her LBP and dissociation in the pelvic region. |
| Report briefly on each Kosha below | Progress toward wellness or worsening reported by the client/group or that you observed in the following areas |
| Physical level | LBP and neck tightness eased with stretching and strengthening. Some jaw tension was relieved with self-massage and extended exhale. |
| Energetic level | The client learned techniques to energize when feeling fatigued and techniques to calm when feeling agitated or anxious. |
| Emotional | The client also gained tools to uplift her mood, shifting out of a tamasic to a more balanced, saatvic state. |
| Intellectual / Sense of self | The client is curious about both the science of yoga and the ancient wisdom behind it. She is making strides toward accessing her true spirit, being consistent with her meditation practice. |
| Spiritual orientation and support plan | Being out in the expansiveness of nature broadens the client’s perspective and has begun to provide her with a connection to something bigger than herself. |
| Additional Information | |
| Feedback received from client/group, anecdotal or written | The client was grateful that our work together gave her the tools and support to establish an adaptable, more consistent home yoga practice. |
| Sample of homework given between sessions (after initial homework) | This is a selection of suggested home practices. Breath and awareness practice. Ha breath, extended exhale, breath of fire, breath of joy. Pavamana mantra/Om Asatoma. With an explanation and Spotify link. Moving meditation. This can be practiced seated or standing with a rocking motion. It can be a nice calming practice preparing for sleep. Svadisthana Mudra. Mudra of the 2nd chakra, the sacral chakra. Element is water. Color is orange. Supine. Place the right hand on the low belly, front of pelvis; extend the left arm out to the side with the palm facing up. Invite the breath to be smooth, steady, and easeful. You might sense receiving universal energy through the upward facing hand, flowing into the sacral plexus. And, you may sense a release or openness in this area, letting stuck energy flow away, along the area. |
| Personal reflection from doing client/group. | |
| Rough estimate of time spent in preparation and follow up documentation per session | To prepare: 30-45 minutes. To document and prepare email correspondence with client: 60-75 minutes. |
| What you would change with benefit of hindsight | Ask more questions of the client. Be nimbler in my responses when the unexpected presents itself. |
| Questions, problems, areas in which you’d like more support | I could benefit from more practice with motivational interviewing, including role playing. |
| Did you enjoy your service? | Yes, most certainly. |
