Activities

It has been three weeks since we last met, as Mack was recovering from COVID and experiencing symptoms such as shortness of breath and fatigue. He has emailed me to say he is ready to resume the session.
He reported that over the past week, he practiced several exercises including heel drop exercises, heel-to-toe walking, and alternate nostril breathing. Heel-to-toe walking has been challenging for him. I suggested that he ask family members for support with this exercise. He reported experiencing body pain when getting up in the morning, so I suggested doing gentle stretches in bed to help alleviate the discomfort.

I began by leading a few minutes of three-part breathing to relax both his body and breath. Then moved to physical practices that included neck and shoulder stretches combined with slow breathing, JAPA, and balancing exercises. I demonstrated some gentle seated poses that he can practice in bed. To help him relax further, I offered a session of yoga nidra while seated in a chair and we practiced alternate nostril breathing together.

Client/Group progress summary

Mack reported that deep breathing is helping Mack to go back to sleep after waking up several times a night.

One leg standing while holding a countertop is improving his balance gradually.

Reflection and self-evaluation

My other client has stiff person syndrome and other conditions, but this is the most challenging case for me so far. I believe he moves more, and he feels better.
I added some gentle stretches in the bed, he appears to enjoy them and is trying his best

Final Client/Group ReportAfter seeing your client/group (for at least 4 sessions including interactive intake)
Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

I will continue to follow this care plan for the next several sessions as he can execute most of his homework assignments and they are working well.
