  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.

  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)

Homework assignment to client/group

1. Balancing Exercise (one leg standing, heel-to-toe walking, hip abduction)

2. Lower body muscles strengthen exercises

3. Eye movement: 1 or 2 times a day and as needed.

4. JAPA Meditation with Alternate Nostril Breathing, suggested combining his 10-minute daily practice with the affirmation, “I trust my body’s ability to heal and restore itself,” which became his favorite mantra.

Activities

I began the session with a check-in where Dan mentioned no major changes since the last session. He reported that there have been no major changes since the last session. He reported he completed the nerve mapping procedure, describing it as using electric shocks and a very unpleasant experience. He will have to see his family doctor to discuss further treatment, but he expressed that he does not want to take any more pills. Also when I asked about his choking during meals, he replied that he would need to see a specialist soon.

I led a 3-parts breathing to relax his body and bring his mind away from his negative thoughts by his unpleasant procedure. Then, we moved to physical practice, including neck/shoulder stretches and JAPA on the floor. As he became more comfortable on the floor, I introduced seated poses such as a gentle spinal twist and knee-to-chest pose. For standing poses, we did one-leg standing for balancing, hip flexion and extension, and abduction. He liked to stretch his lower back by the standing forward bend, supported by a chair.
A 15-minute yoga nidra helped him to relax physically and mentally to end the session

Client/Group progress summary

He reported his lower back is more flexible now.

Dan's wife now commutes to her office, leaving him alone at home in their two-story house. He takes care of the household activities and frequently uses the stairs. The homework assignments and daily activities are helping him make progress

Reflection and self-evaluation

He seemed to be frustrated today when he spoke about the nerve mapping procedure and his choking conditions. I will carefully observe and provide additional mental/emotional support as needed.

He is comfortable with homework assignments and showing progress, this care plan is working well.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. I will continue to follow this care plan for the next several sessions, especially focusing on physical strength to improve his balance.
