| Activities | Karen skipped a session the previous week because her son required extra care due to complications following his spinal surgery. I began the session by checking in to see if there were any physical or mental changes. She reported improvements in lower back pain (LBP) and sleep quality. Additionally, she shared that she believes she has a fear of getting up from the floor, as I had asked her to prepare some props to practice on the mat in advance. Due to her hip pain and being overweight, she had not been on the floor for many years. Now, having lost weight and undergone hip replacement surgery, she is more mobile. I demonstrated how to get up from and down to the floor. She mentioned that she will practice seated stretches with her husband. I let brief guided meditation to bring her to the present moment and relax her breath. For physical practice, I led chair stretches including spinal twist, knee to chest, forward bending and seated cat-cow poses. And I demonstrated again floor stretch of child pose, knee to chest, spinal twist, and cat-cow, reclined butterfly. We ended the session with 15 minutes of yoga nidra in the chair to relax the body and calm the mind. |
| Client/Group progress summary | She reported feeling good during savasana on the floor and noted that her lower back pain has improved. She was able to reduce her pain medication dosage. Additionally, her sleep quality has improved since she began practicing alternate nostril breathing before sleep. |
| Reflection and self-evaluation | Her fear of getting up from the floor was somewhat surprising to me because I had never encountered this before, but it is understandable, and I am glad she brought it up. This was a valuable experience to me, and I learned another area to pay attention to. |
| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |
| Plan for next session | I will follow this care plan and hope she can practice on the mat for the gentle stretch series. |
