Homework assignment to client/group

Below are the home practice assignments, the progress will be assessed, and new practice may be added accordingly.

• gentle shoulder rolls and movements
• neck movement practice using visualization
• daily inspirational reading and reflection
• alternate nostril breathing / brahmari breathing
• supine butterfly, supine spinal twist, knee to chest pose, child pose

Activities

I began the session by asking if there were any changes physically and mentally. Arnette reported that her neck pain remained unchanged, and she sounded frustrated. I explained that being aware of one’s posture is very important in our current lifestyle, particularly in activities like texting and keyboarding. She described her posture as particularly poor while driving, writing, and reading books in bed. I suggested that she assess her posture during her daily activities and correct it if necessary, also mentioned using props are helpful.

I led 3 minutes of 3-parts breathing practice as centering to bring her mind calm. Next, we moved to asana practice, including gentle shoulder movement practice in the chair followed by seated asanas, supine spinal twit, knees to chest, and supported child pose.

To end the session, I invited her to 15 minutes of yoga nidra followed by brief meditation to relax to physical body and calm her mind.

Client/Group progress summary

Daily shoulder stretches help alleviate her upper back and upper arm pain, and breathing practice is effective to manage her stress.

Reflection and self-evaluation

Arnette is still suffering neck pain caused by an accident after 10 months of PT.
From what she says, she seems hesitant about new treatments therefore she relied on only pain medication.
She is comfortable in the gentle stretch with deep breathing except for neck flexion, this plan is improving her condition gradually.

Final Client/Group ReportAfter seeing your client/group (for at least 4 sessions including interactive intake)
Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

I will follow this care plan. I will offer guided visualization targeting reducing the pain.
