Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them. Your care plan proposal should be approved by the mentor before session 2 if possible, or 3 if approval is delayed by mentor.

Activities
Questions?
Movement and Pain Assessment
Lower body pins and needles
Right upper body Shoulder lat flex approx. 110 degrees 80 deg flex, can write messy, elbow 90 plus.
Left Upper Body Hand extra sensitive Shoulder joint is injured since? It seems to be dislocated. He has seen people, but no one has assessed and gotten imaging. I believe he is wearing clothes that you can see the malformation in the joint. Elbow and wrist hardly able to move today. He had some Chinese food and it reinflammed his left arm. He was able to move elbow as much as I have seen, but wrist was more impaired than normal.
Right lower body Super sensitive with pins and needles (again due to food). Side lying, he can lift his right leg up an inch off the other. His right leg he can bend and move. Movement is progressing. Ankles and toes all good.
Left Lower body His Leg can lift off bed from hip an inch, he can bend knee with effort. More movement and flexibility in left leg. Still stiff and weak but improved. Ankles and toes good.
Breath he is able to easily deepen his breath now and ist still learning to use deep and gentle breaths with effort.
Mental He has noticed increased loss of short-term memory.
Emotional He enjoys employing visualizations for those times he feels vulnerable. He enjoys putting on Ram Dass to lift his mood.
Wisdom He feels better connected to his self that he relates to being there all the time. He knows this is a self-growth moment.
Spiritual He wishes to be better connected to nature as he was before living at the cabin
He did bridging and the side-lying leg raises.
Client/Group progress summary
After this assessment, a few things are more clear. He has a shoulder that is clearly not correct. He had a hoist that they used to get him to toilet, he remembers hurting his shoulder months ago. He was in a lot of pain til the last couple months but his ROM is not improving and the pain on movement is very intense.
His eating Chinese food led to sensations, edema, and pain.
Overall he has been making progress. I'm unsure what the outcome will be here, unless he gets his specialists on board.
Reflection and self-evaluation
I enjoy working with this client but he needs to take some responsibility to make steps forward towards walking and movement in his shoulder. He needs the shoulder to be able to use a walker.
Final Client/Group Report
After seeing your client/group (for at least 4 sessions including interactive intake)
Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
