Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Homework assignment to client/group

1. Client to use guided meditation at night (recording) before bedtime.

Activities

Reviewed with client her current symptoms, the most prominent of which is sleep. Discussed various ways we could look at helping her feel calmer at night. Client with interest in making her own guided meditation. Decided to start with this practice.

Using Stress Management manual asked client what her most calming setting is. Asked her what smells, sounds, tastes, feelings, she had surrounding this place. Client chose to be at the beach with no one else around. Helped her construct a guided meditation script in session.

By the end of the session, she read it and recorded herself on the her cell phone.

Client agreed to use this at night before sleep; agrees to put cellphone in airplane mode and silent mode before sleep.

Client/Group progress summary

Client, thus far has not had much improvement, but she is open to yoga and its power to help her.

Reflection and self-evaluation

The client is very friendly and also quite anxious. It will be interesting to see how she responds to yoga nidra practice. I will start only with a body scan and see how she does with that for the first few weeks then consider yoga nidra.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

2. plan to teach body scan and possibly JAPA practice with focus on the shoulders/neck/back.
