6. Gentle stretches in bed before getting up

7. Neck/shoulder stretch - neck movement needs to be very careful

Activities

I began today's session by asking Mack how he was feeling and if he had noticed any physical or mental changes or challenges since our last meeting.
He reported that he continued his daily practice and saw some improvement, he walked about 7000 steps on Sunday at Central Park but felt a minor pain on his right hip. I suggested paying attention to the overuse of sudden increase of hips knees and ankles. He also reported that he woke up 3-4 times every night due to his sleep apnea, but alternate nostril breathing is helping him to go back to sleep. He tried using C-PAP device while he slept but he stopped using it because it did not improve his sleep.
Then I led a few minutes of guided meditation. Today’s physical practice began with gentle neck/shoulder movement followed by joint freeing practice, then balancing and muscle strengthening practice, he was able to do heel-to-toe walking more than 12 feet with my support, one leg standing, and heel dropping.
Yoga nidra and alternate nostro breathing helped him relax his physical body and mind.

Client/Group progress summary

Mack reported that deep breathing helps him to go back to sleep after waking up several times at night. His balancing is gradually improving as he can stay 25sec/R, and 14sec/L with one leg. He can continue heel-to-toe walking about 12 feet now.

Reflection and self-evaluation

Mack is a physician, and he knows how to measure his improvement. He measured his balance on his own, which I did not ask him to do. I am looking forward to hearing about his improvement at the next session. The plan is working well especially improve his physical body, and his energy level will be increased as well.

Final Client/Group ReportAfter seeing your client/group (for at least 4 sessions including interactive intake)
Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

I will continue to follow this care plan for the next several sessions to improve his balancing and strengthening muscles. Additionally, I look into improving his quality of sleep.
