Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Homework assignment to client/group

• Three-part breathing and meditation 15 minutes daily
• 15-20 minutes daily yoga -joint movement, child-table-downward dog, sun salutation, cobra, hand to big toe, bound angle pose, leg up the chair and savasana
• Keeping energy log– how she feels (from scale 1 to 5), time of the day and what she is doing or not doing at the time.

Activities

The client has a history of lower lumbar disk compression (bulging disk), now generally reduced flexibility, especially of the hips and knees. The session focuses on increasing flexibility and building upper body and core strength.

Client/Group progress summary

The client reported that her flexibility and upper body strength have improved. The Global health assessment also indicated improvement of physical and mental health. I also noticed that the client’s upper body strength has improved compared to the first session. Seated forward bend was still challenging for the client due to her past lumbar injury. The client accepted her physical limitation. Although the client felt that her fatigue level has not changed, the fatigue score of the Global health assessment changed from severe to moderate.

Reflection and self-evaluation

The client’s stamina appears to improve from when we started working together; downward dog is not as shaky as before. During the past few sessions, she did not look exhausted when she arrived. Therefore, I was a bit surprised that she did not feel any improvement in her energy level. She has low in the morning and then afternoon. I am wondering if this is because of not having enough sleep (she reported that she felt better this morning after a good sleep) and a natural energy dip in the afternoon. I hope that keeping energy log will help her understand the pattern, plan her daily activities and identify activities that drain her energy.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
