Homework assignment to client/group

Continue with breathing practices, especially extended exhalation and alternate nostril breathing.

Activities

The client has been diagnosed with stage 4 cancer; she also has chronic inflammation.
• Checking in – assessment PEG pain screen tool and FSI
• Centering – witness practice
• Three part-breathing
• Guided imagery
• Meditation – positive thought (peace)

Client/Group progress summary

PEG assessment indicates that the client can cope with pain because the pain level remains the same but does not interfere with her enjoyment of life. FSI score improved slightly from the first session. I can also see that she looks better (brighter and has more energy than when I met her the first time). She reported that she has been doing breathing practice more than before, although she does not have formal practice. She feels that breathing has helped her, especially to manage stress and pain.

Reflection and self-evaluation

Originally, I planned to work with the client for at least 8 sessions. But because of the lack of commitment to do home practices and adhere to cancellation policy, I feel that I need to end therapeutic relationship with the client. Continuing working with the client in this capacity will not benefit her. I discussed with the client about this, and she agrees. I still want to support her, but as a friend, (she was referred to me by a mutual friend). Although I ended the relationship sooner than I expected, it still is a good learning experience for me.

Final Client/Group ReportAfter seeing your client/group (for at least 4 sessions including interactive intake)
Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
