Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them. Your care plan proposal should be approved by the mentor before session 2 if possible, or 3 if approval is delayed by mentor.

Client/Group progress summary
She was feeling better today, she is improving every day. She said she felt some pain early today but rested a little and the pain was gone.
She went to her primary doctor and asked some questions. I told her to check her thyroid and she did and to her surprise, her TSH was very high. She asked for a referral to an endocrinologist. Maybe her symptoms (Afib, muscle spasms, not gaining weight, fatigue) could be related to a thyroid malfunction.
She did journaling inspired by the saying from the article about suffering:
“The moment you know how your suffering came to be, you are already on the path of release from it. —Thich Nhat Hanh
She said meditation and self-inquiry (swadhyaya) help her uproot some deep stuff.
We discussed the 1st lock and key and will continue going deeper into the subject in the next session.
She is more comfortable with her asanas and memorized the sun salutation on the floor.
She does all the asanas on the floor and only sits on the chair for twists. We tried a supine twist, then she didn’t have to move from the floor. Although her legs were supported with a block, because of her fused spine, her shoulder couldn’t reach the mat during the gentle twist, but she felt ok doing that.
Reflection and self-evaluation
I am very glad to see her improvement.
Final Client/Group Report
After seeing your client/group (for at least 4 sessions including interactive intake)
Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
