Tools from each module should be used (not on each client – but overall) |
| Session Date | 08/29/2024 |
| Session Number | 14 |
| Total Session Minutes | 75 |
| Homework assignment to client/group | 1. Balancing Exercise (one leg standing, heel-to-toe walking, hip abduction) 2. Lower body muscles strengthen exercises 3. Eye movement: 1 or 2 times per day and as needed. 4. JAPA Meditation with Alternate Nostril Breathing, suggested combining his 10-minute daily practice with the affirmation, “I trust my body’s ability to heal and restore itself,” which became his favorite mantra. 5. Seated gentle stretch 6. Gentle neck/shoulder stretch |
| Activities | He reported that he had not practiced for days due to his esophageal dilation procedure 6 days ago. He previously stated it took a few days to resume normal food after the procedure. Despite his fatigue and stress, he continued JAPA meditation and breathing |
| Client/Group progress summary | I have seen Dan’s big progress in his physical practice since we began the session. He started with seated joint freeing and gentle shoulder stretching, I added more practices into his routine such as balancing, muscle strengthening, and seated poses. Now he has a complete series of yoga practices to improve his physical health. |
| Reflection and self-evaluation | Dan's motivation and dedication of recovery from cancer surgery and the side effects from chemotherapy have significantly contributed to his progress. Gradually adding new exercises was a better choice, and the plan shows good results. |
| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |
| Plan for next session | I will continue to follow this care plan for the next several sessions, additional asana will be introduced gradually. |
| Report briefly on each Kosha below | Progress toward wellness or worsening reported by the client/group or that you observed in the following areas |
