Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Client/Group progress summary

We've been working together since April and have made good progress. It's time to wrap up and we're in agreement about that. NM's commitment to her wellness, in face of the various health conditions she's facing, has been impressive. Going forward I trust she'll continue her path of yoga and mindfulness and learn to appreciate and cherish herself more. She is enough and she's doing more than enough!

Reflection and self-evaluation

I've enjoyed working with NM. Wasn't always sure I was getting through, but I do look forward to wrapping up with her trusting she'll find her way forward whatever she chooses.
I've learned lots from working with her, from trusting my gut to creating space for her to open up more. We've had some fun and laughs along the way.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

This will be our final session, so I plan to do an overall review and reflection of where we started to where we are now. Will be sure to emphasize Yogic principles including Ahimsa for herself, Saucha for keeping things simple and Upeksha for being with whatever is showing up and using her tools to help her mindfully respond rather than react. Will also remind her about the breath and how it's always available to bring her back to present moment.
