Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

• Three-part breathing and meditation daily 20 minutes
• Establish morning and bedtime rituals that work for her (e.g., set a timer to put down her phone and do breathing and meditation before bed).

The client has stress and anxiety related to taking care of a 94-year-old friend who is the mother-in-law of her late husband.

Client/Group progress summary

She slept better than she was. Although it is still challenging, she has become more intentional about stepping back and letting staff at the assisted living facility and health care professionals assess her friend’s physical and mental condition.

Reflection and self-evaluation

The session focused on self-study and surrender. I explained Yama and Niyama and discussed Svadyaya and Isvara Pradnidhana. The practice portion also emphasized this teaching, e.g., letting go of the physical body (notice of what she can let go when holding a pose), thoughts and ego. I feel that the client is beginning to understand that she needs to step back and not forcing what she thinks is best to her friend.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

I will continue to bring yogic wisdom to the session. She likes to walk; so, I will also bring walking meditation to the session. The asanas and pranayama will focus on grounding.
