Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

1. Meditation – 5 min daily

Activities

• Check in and assessment – balance (standing on one foot and tandem standing) and mood
o 4/5 client has excellent move scale (5 out of 5)
o 1/5 has a mood scale of 3, she was a bit stressed from a upcoming trip and just recovered from a sinus infection
o For balance
• Client#1 has excellent balance; she could stand on one leg and tandem standing for 30 seconds, although tandem standing was easier than standing on one leg
• Client #2 had no problem with tandem standing, but find standing on one leg more challenging – she
could only stand on one foot for ~20 seconds
• Client #3 could do both for 20 seconds
• Client #4 had difficulty in standing on one leg (could hold for 5 seconds), but did better with tandem standing, although she could not hold for 30 seconds
• Client #5 could hold the two positions for only a few seconds.

Client/Group progress summary

This is the first session; therefore, I cannot say anything about progress yet. The clients are different in their physical limitations, strength & balance and experience in yoga.

Reflection and self-evaluation

This is my first group yoga therapy session; so, I was a bit unsure on how to start the class. I explained the format of the class, did an assessment and discussed the theme of the session, i.e., mindfulness and being at the present moment. I explained the benefits of each movement (e.g., why moving the joints is important as we age), instructed the client to watch the body and breath. Because each client has different levels of yoga experience and mobility, I offered multiple options (using the term ‘bus stop’ 1, 2 or 3) and I do the easiest option with the clients. One client did reverse breathing; therefore, I taught a basic three-part breathing to the class and explained the movement of the diaphragm during inhalation and exhalation (to keep clients with more advanced experience engaged). Overall, I think that it went well.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

I will add a few strengthen poses, e.g., warrior series, triangle, plank. Pranayama will include three-part breath and alternate nostril breathing. I will discuss klesha in the next session.
