Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)

Homework assignment to client/group

1. Balancing Exercise (one leg standing, heel-to-toe walking, hip abduction)

2. Lower body muscles strengthen exercises

3. Eye movement: 1 or 2 times a day and as needed.

4. JAPA Meditation with Alternate Nostril Breathing,
suggested combining his 10-minute daily practice with the affirmation, “I trust my body’s ability to heal and
restore itself,” which became his favorite mantra.

Activities

He reported he saw his ear-and-throat doctor a few days ago, and the doctor found that acid reflux impacts his vocal cords. He abstains from eating for four hours before bedtime and uses 3 pillows to manage the condition. He is still experiencing a cough while he swallows his meal.
I began the session with guided breathing practice followed by eye movement.
Then we moved to asana practice, beginning with neck/shoulder stretch and joint freeing,
standing asanas including tree pose for balancing, and chair pose for strengthening lower body muscles. He reported that chair pose was very challenging but will try to practice every day. We completed the asana practice with 3 seated poses.
15 minutes of yoga nidra helped him to relax physically and mentally followed by brief silent meditation.

Client/Group progress summary

He has shown significant progress since we began our session and continues to show gradually. His balancing has been improved but he still needs to work on it. His daily stretch reduced his upper back pain where he had an incision at his surgery and his doctor told him nothing can be done.

Reflection and self-evaluation

I see his progress when I see him weekly, however he reports on the conditions he experiences outside of our session time. He is still suffering from choking and coughing, a painful routine procedure to expand his esophageal. He is less stressed and can control his mind better now, but I am exploring the practice I can offer to support his emotion better. Because I am hearing the challenges in his new life after cancer surgery more than 2 years ago.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

I will continue to follow this care plan for the next several sessions, additional asana will be introduced gradually to strengthen his physical body.
