Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.
Your care plan proposal should be approved by the mentor before session 2 if possible, or 3 if approval is delayed by mentor.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

I didn’t need to make significant modifications to my care plan for this client. Once she received clearance from her doctor, she was ready and motivated to regain movement in her right hip. She approached our sessions with dedication, taking her practice seriously and diligently completing her homework. I would consider her a model student in her commitment to the process.

Client/Group Goals

She has made significant progress toward her goals, demonstrating full range of motion in her right hip, minimal pain, and improved flexibility compared to before her surgery. With continued movement and practice, she will likely see even greater improvements over time.

Physical level

At the end of our time together, I noticed a significant improvement of her right hip. She was able to regain full range of motion with little pain.

Energetic level

My client was always cheerful and ready to work, consistently showing up with a steady, moderate level of energy. Her positive attitude and dedication made each session productive and enjoyable.

Emotional

My client was eager and optimistic when we met, always approaching life with a glass-half-full mindset.

Intellectual / Sense of self

My client has a strong and clear sense of self. As a Juilliard-trained opera singer, she carries herself with confidence and ease. Her experience performing on stage in front of hundreds of people reflects her deep self-awareness and comfort in her own skin.

Spiritual orientation and support plan

My client is deeply spiritual and highly family-oriented. She maintains a regular meditation practice and actively engages in personal growth through workshops and retreats, demonstrating her commitment to both inner peace and meaningful connections.

The client was very happy with her results. After each session, she expressed feeling stronger in her hip, reflecting the progress she made throughout our time together.

I’m not sure I would do anything differently. I feel we progressed at the right pace, allowing for steady improvement without feeling rushed.

I truly enjoyed working with my client. Her infectious positive energy made each session uplifting, and it was impossible not to feel happy in her presence.
