The outline should show the sequence of practices as you plan to offer them. Your care plan proposal should be approved by the mentor before session 2 if possible, or 3 if approval is delayed by mentor.

Client/Group progress summary
Client has been sick with a cold for the last two days and did not slept well last night. She has been concerned that she will be behind schedule for studying.
Client reports that her heart rate does not increase because she does the asana before the meditation. She is doing the asana before she studies.
Client reports that she is doing diaphragmatic breathing (Dirgha Swason) in between her studies. Client mentioned that many people use smoking to calm themselves down but it has negative consequences and it’s a very short relaxation. And that the breathing she is doing is “coming from the inside and works better than smoking - its clears the mirror and connects to her soul, she feels complete in herself” to calm herself.
Client reported that she doing Asana when she needs it usually in the middle of the day. She is also trying to do asana with her son, he imitates her. Its not very concentrated doing it this way but there is lots of laughter.
Client says she wants to do her practice everyday at the same time and the same routine. She doing Asana when she needs which is usually in the middle of the day.
I wanted to check-in with client in this session about some of the things she had talked about in the past that were obstacles for her. I asked her about how her sleeping is doing?
o Client reported that she takes an anti-histamine to sleep every night and it puts her to sleep and keeps her in sleep.
Is she still shopping on the cell as a distractions?
o Client reported that she is doing ‘zero’ shopping – not doing anything.
o Client said that she is trying to keep everything more productive and under control. She is only doing the necessary things. Keeping track of time and be organized, and doing just what is the priority.
Is she eating food as a way of distracting herself?
o She is not doing this. The Ritalin that she takes also reduces the appetite. No snacks, no junk food.
How is the rumination?
o “Much less.” She is trying to make better use of her time now. When something happens yes there is rumination, and also things related to her exam. This is when she starts the diaphragmatic breathing. And she is practicing being present.
Reflection and self-evaluation
I was about to close the session and I had forgotten to chant Hari Om with client. Client reminded me that we didn’t do the chanting this time and she likes it. I thanked her and we did it.
From the responses that the client gave to the questions in the section above, I find that client is operating more now from her Vijnanamaya Kosha. She is thinking and discriminating and being more practical, and not being as emotional as she used to be.
Final Client/Group Report
After seeing your client/group (for at least 4 sessions including interactive intake)
Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
