Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Homework assignment to client/group

- SHOULDER ROLLS - roll shoulders together or separately UP / BACK / DOWN 3-5 X. Then reverse - BACK / UP / DOWN 3-5 X.
- BALANCE - using a stable chair - where it can’t move, stand next to the back of the chair -
— LEFT SIDE - place left hand on chair, as needed, lift your left knee - hold for count of up to 10. Release.
— RIGHT SIDE - place right hand on chair, as needed, lift your left knee - hold for count of up to 10. Release.
- BELLY BREATHING - take one or two pauses during the day - sit w/ hands on belly for 3 to 5 minutes

Client/Group progress summary

This was our Intake session. We covered a lot of ground. Client loves to interject stories during our time together. He was most appreciative of all we covered.
He leads a mostly sedentary life, which has contributed to his Dropped Head Syndrome, 2016 knee injury and balance issues. He reminded several times that he used to be very athletic.
He's very unsteady with his right knee - getting up from a chair and climbing stairs, which makes him a fall risk. Balance and leg strengthening (chair leg lifts / gentle squats w/ chair) will be a focus in our work together.

Reflection and self-evaluation

All in all, I believe practiced good listening. Patience too - with client's interruptions to share a story and connect. I was delighted to see my intuition show up as I improvised several times during our session - suggesting movements and asking questions.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

Based upon our first session, I'll develop my Assessment sequence - focusing on range of motion for his upper body and determining baseline for balance by timing each standing pose.
