Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Homework assignment to client/group

- SHOULDER ROLLS - Sitting in your chair -
- Grounding into your feet and your sits bones on your chair
- Gently engaging your core and lifting through your sternum
- Roll shoulders together or separately UP / BACK / DOWN 3-5 X. Then reverse - BACK / UP / DOWN 3-5 X.
- BALANCE - using a stable chair - stand next to the back of the chair -
— LEFT SIDE - place left hand on chair, as needed, lift your left knee - hold for count of up to 10. Release.
— RIGHT SIDE - place right hand on chair, as needed, lift your left knee - hold for count of up to 10. Release.
- STRAP - WORKING WITH ARMS & SHOULDERS MOBILITY
- Standing or sitting with feet grounded and hip-width apart
- Standing w/ arms relaxed at your sides or sitting with hands on your thighs, take hold of strap with hands shoulder-width apart
- Inhale extending arms out in front of you, lifting up skyward as far as you comfortably can. Exhale return hands to starting position. Repeat 5x - 10x.
- BELLY BREATHING - 3 to 5 minutes daily

Client/Group progress summary

Client is very open to our work together. Provides clear feedback. Seems to be straining at times. I remind him to let the breath guide his movement and limits. If breath becomes ragged or uneven, it's a sign to back-off, slow down or stop. He hadn't practiced much between sessions - was apologetic.

Reflection and self-evaluation

I'm pleased to see that client is finding definite benefit from the tools and practices I've shared with him. Feel as though I'm tuned in and have a good sense of where we're headed and what would be most beneficial to him.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

More of the same. Fine-tuning strap work and chair asanas. Talking more about stress management and looking for ways to introduce more substantive Raja principles, including Ahimsa and Santosha.
