Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

• Three-part breathing and meditation 15 minutes daily
• 15-20 minutes daily yoga -joint movement, child-table-downward dog, sun salutation, cobra, hand to big toe, bound angle pose, leg up the chair and savasana
• Keeping daily log – energy levels, activities, diet, sleep quality

Activities

: The client has a history of lower lumbar disk compression (bulging disk), now generally reduced flexibility, especially of the hips and knees. The session focuses on increasing flexibility and building upper body and core strength.

Client/Group progress summary

The client reported that she feels stronger and more flexible. Her energy level has already improved. She went hiking with her friends last week and did not feel exhausted as she was before starting yoga therapy. Her mental health has improved as well.

Reflection and self-evaluation

I can see that the client’s physical and mental health has improved as soon as I saw her getting of the car. She becomes more aware of her body and energy level and recognizes lifestyle choices that make her feel good (healthy diet, enough sleep, regular exercise). She commits to the home practices and the result of her effort is clear. We discussed the importance of consistent effort, and yet not attaching to the outcome.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
