Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Client/Group progress summary

Progress continues. Client had an intake appointment w/ her new PCP. Nurse came in first to take vitals, etc. When nurse measured her height, it was 5' 4". Client then remembered work she'd been doing on her posture and stance. Asked nurse to check her height again. She grounded into her feet - focused on inner arches, engaged core as she lifted sternum and both sides of neck lengthening through crown of head. Nurse updated her chart to 5' 5.5".
Client forgot to mention kyphosis diagnosis with new PCP. Said she'd do so during her next visit.

Client is under a lot of stress as her husband, 63, has been displaying signs of cognitive impairment, including one alarming incident while he was driving, which resulted in police pulling them over. She's understandably worried about what this might mean for their future.
She expressed gratitude for her practices and remembering to rely upon them as they determine what's next for her husband's well-being.
Client needed to talk a lot - I sensed her need for me to listen - so we didn't get to all that I had planned for our session.

Reflection and self-evaluation

I'm glad that our connection is well-established and that I can meet her in the moment wherever she's at, especially when faced with serious matters.
There's a part of me that feels I have to keep things fresh and exciting, yet another part that senses it's best to go slow and support client to deepen into the practices and tools that best support them.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

Continue with our focus on her posture. Start to focus on what she noted on her Intake Form, Energy Management. We'll review the Gunas and how breathing practices, whether energizing or calming, can support her with Energy Management.
