Approval Notice
Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Homework assignment to client/group

1. continue asana practice (tadasana, hip circles, bouncy twists, and tree pose), going outside, sound healing.
2. client to practice once a day of "Chicken dance" (koppala bhati breath) upon wakening from bed to help her feel awake.
3. continue dance practices.
4. avoid TV and horror movies.

Activities

Spoke with parents about client's overall health. They state she has been helping more around the house; father noted that she seems more like her usual self. Noted coughing in this session, when speaking to parents about her symptoms.

Client has been more able to participate in activities of taking care of the home. No socialization with others outside of her parents except for the day program she attends as yet.

Client danced in session with this writer and BHS. Able to make better eye contact

Practiced tree pose and the rest of the asana/warmup.

Client brought out her Pikachu, stuffed animal to show.

Spent most of session with her parents in the other room.

Client states she is feeling sleepy when she wakes up in the AM.
Added motions to arms and feet, to koppala bhati breath and practiced with client. "Chicken dance"

Client/Group progress summary

Client is gradually, more interactive, more interested in starting the dance practice on her own; she is genuinely excited to have our sessions. Her mood has improved.

Episodes of agitation seem related to psychotic symptoms. Possible client will need a change in medication but her parents would prefer not to do so at this time.

Reflection and self-evaluation

I feel I am learning a lot with this client and able to interact with her in a way that is helpful.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
