| Homework assignment to client/group | 1. client to use mantra (I am good.) when she has negative thoughts about herself. |
| Activities | Received report from family that client has improved; decrease in hallucinations; one episode of agitation since last session. Mother states client has been helpful at home with household chores. She still cannot tolerate being in large groups of people. However, anxiety and levels of agitation have improved since last session. Client was in good spirits today. She brought out her stuffed animals to show to us. She likes Pokemon and was showing Pikachu. She was present on the screen and engaged predominantly without any interruption from her parents. Parents, after giving report of her symptoms, allowed her to have her session. Father, prior to leaving screen, was very thankful to have his daughter look more like her usual self. Client danced; she talked to the behavioral health specialist about planning to go out on the weekend to buy a new puzzle. She showed us her most recent puzzle that she has put together. Has been doing puzzling every day in the morning in a meditative fashion. Taught client mantra, "I am a good." Phrase was kept very simple and client both chose and agrees with this phrase. Client agrees to repeat this mantra every time she has a bad thought about herself. In this way, she can hopefully remember that even when she doesn't feel like a good person, she will always be; and the yoga therapy space we provide will continue in her inner world to be a space where she is good. |
| Client/Group progress summary | There is progress being made with regard to client's perception of herself and a decrease in number of agitated episodes. |
| Reflection and self-evaluation | Thus far, I have learned how important it is to preserve a regular space for this client to help shape her own mind. |
| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |
| Plan for next session | 1. review homework and mantra. |
