Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Homework assignment to client/group

1. mother and client to attempt to decrease meat in diet and to eat more vegetables and fruits.
2. client to continue to review affirmation card; continue to do mantra when she has negative thoughts about herself; continue asana, continue outdoor walking and dance and sound healing.

Activities

1. client pulled affirmation card "I trust and know that all is well in my world." Client expressed understanding of this concept. She listed things that were going well in her world.
2. agrees to review this affirmation card daily.
3. reviewed asana.
4. mother and father gave report that she had zero episodes of agitation noted. No hallucinations. Still cannot tolerate being around groups as yet.
5. reviewed dietary recommendations with mother; client has been eating predominantly meat (chicken) daily at multiple meals. Encouraged a balanced diet that includes healthy options which would be culturally appropriate and to include sufficient fruits and vegetables.

Client/Group progress summary

Client continues to improve. She has taken well to affirmation cards. I think the fact that they are in English, which is a world her predominantly Mandarin speaking parents cannot partake in, is helpful. She is gradually more able to see herself as a separate being.

Reflection and self-evaluation

I myself really like these affirmation cards and they help instill in both the client and parents a positive perspective. I myself will try these affirmation cards to see if they help my own mind.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

1. pull new affirmation card.
2. review asana practice and dietary intake (mom has agreed to let me know if any dietary changes were able to be made).
3. continue to address parent child dynamics and encourage the separation of the disease from the client's true nature.
