Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Homework assignment to client/group

1. continue breathing practices before dinner and other practices that have been assigned.
2. clients to bring in object to use for next session.
3. clients to listen to my yoga nidra recording (from Giovanni's phone) before bed.

Activities

1. opened with diaphragmatic breathing and sound healing.
2. did check in with talking object and conscious listening.
3. clients discussed their work and school activities and how practices have been going.

4. took brief break, then started did crystal demonstration with food coloring; all clients expressed understanding of the concepts.

Clients were taught about the mind body connection as well. Taught body scan. After this entered into a gentle JAPA/chair yoga practice using info from Stress manual . Started with upper body and worked our way down. Clients remained seated.

1. grounding breaths, gentle lateral rotation of the neck (looking to each side of the body) about 5 x per side.
2. reaching ear to shoulder, slowly and gently (5x each side)
3. chin to chest (5x)
4. looking up to ceiling (without dropping head back) (5x)
5. eye movements with gentle massage of temples at the end
5. arm raises (front, side) 5 x each side
6. shoulder rotations forward and backwards, shoulder shrugs
7. wrist rotations (5x forward and backwards and on each side)
8. seated baby cobra pose (3x)
9. hugging self and holding for about one minute, with option to walk hands down thighs, knees and shins to toes to go forward bend, then gently back up.
10. used chair to gently rotate torso to each side, using back of chair and chair's seat to hold for support.
11. lifted thighs gently bilaterally (5x each)
12. extended and flexed knees
13. rotated feet together in one direction and then another around the ankle joint; did windshield wipers of feet bilaterally.
14. flexed and extended toes, both sides the same time.
15. did yoga nidra practice with clients lying on the floor.
16. used sound bowls for periods of silence.

After this, session was ended; clients aware to do this brief practice when they feel called to do so. This aspect of the session (chair yoga was audio recorded by one of the family members on his phone) and family agrees to use this recording to do chair yoga at home.

Client/Group progress summary

Mother reported attending all of her work days this past week, along with Sara. Giovanni and Julia also report feeling calmer during the day. No episodes of aggression or anger reported by Giovanni this past week. Mother has continued to check in with her therapist and reports feeling safe in this setting.

Reflection and self-evaluation

Today was a more practice based day; I felt the clients really wanted this after last session which was emotionally loaded. Overall, I feel more connected to these clients as we do mind body practices together. Clients really liked the yoga nidra. I may attempt to end all the session with a brief yoga nidra practice as this was so popular.

Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

1. will plan to introduce the tradak practice and perhaps another round of yoga nidra.
2. will perhaps add shoulder/neck JAPA practices to each session, as this area was reported to have greatest tightness.
