| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |
| Date you started seeing client/group | 08/05/2024 |
| Total hours of all Yoga Therapy sessions with this client/group to date | 75mins x4 plus 90 min assessment |
| Adjustments and adaptations you made to your care plan, | I plan to continue to see client in order to be able to address her concerns; I had to adapt my plan to use what made the client most comfortable. She liked to use guided meditations so we started with that and then worked slowly toward more subtle practices like the breath. |
| Client/Group Goals | Client with improved sleep after use of yoga nidra practice; reports less daytime anxiety. She feels better in general; easier to wake up in the AM after doing her breath practices. Neck and shoulder stiffness had slight improvement when JAPA was taught, but would have to continue to work with client to see further progression. |
| Report briefly on each Kosha below | Progress toward wellness or worsening reported by the client/group or that you observed in the following areas |
| Physical level | Improvement in sleep and some improvement in neck/shoulder stiffness. |
| Energetic level | improved energy particularly in the morning hours. |
| Emotional | client reports having more positive emotions during the day |
| Intellectual / Sense of self | she has a stronger sense of being something other than her past experiences. |
| Spiritual orientation and support plan | Plan is to continue to work with client; she has the plan to continue her self care routine that we developed. The client is not overtly spiritual, so concepts were discussed in a secular fashion. |
| Additional Information | |
| Feedback received from client/group, anecdotal or written | The client expressed gratitude for the work together; she states she plans to continue to see me. |
| Sample of homework given between sessions (after initial homework) | 1. practice 3 part breathing and alternate nostril breathing. |
| Personal reflection from doing client/group. | |
| Rough estimate of time spent in preparation and follow up documentation per session | 120 |
| What you would change with benefit of hindsight | I would prefer to see the client in person. I would also have focused more on the physical complaints; however, I plan to continue with the client and will continue to focus on the back/neck stiffness. |
| Questions, problems, areas in which you’d like more support | I'd like to delve deeper into yoga nidra; it is clearly very powerful; would be good to see what iRest has to offer. |
| Did you enjoy your service? | Yes. |
