Homework assignment to client/group

1. client to practice stream of consciousness writing at home, daily if possible.
2. continue breath practices and can yoga nidra with or without others.

Activities

Client reports all nights last week being able to sleep and also reports doing to school. Has been feeling more aware of her surroundings during the daytime, as if she is more awake. Reviewed any questions she and mother had regarding yoga nidra practice. They said that they shared the recording with family members and 3 days last week listened to recording from You Tube together.

Both report improvements in anxiety.

With mom out of room, discussed client's table of thoughts/emotions/core beliefs; reviewed breath practices which she says she continues to do in the AM and PM.

Client currently does not do any specific meditation practice aside from yoga nidra. States that she tried meditation with her therapist in the past, but she was not able to focus and she also started to have more intrusive memories. She found these hard to handle.

Discussed nature walking and how walking can be used as a meditation. Would have liked to take client out for a walk and show meditative walking, but since I am using telehealth, I was able to describe how this works. I demonstrated on my end taking slow deliberate steps and being conscious of the surrounding, paying attention to the sights, sounds, feeling of the feet touching her shoes and then the ground as she walked. Client agreed to try this at home; states she lives close to a park (across the street) and she feels safe to walk there.

Demonstrated also a stream of consciousness practice, and client states that she sometimes feels overwhelmed with the thoughts in her head and this can get in the way of sleep and of concentrating on the moment.

Explained that draining the brain was like cleaning out a kitchen garbage disposal; it allows the mind to 'spin' and work more easily and to be harmonious with the true nature/inner energy.

Did 15 minutes of uninterrupted stream of consciousness writing with client; after this, the client felt more at ease. States she will try to use this when she feels overwhelmed at home and school.

Client reports having a new romantic relationship. Described that this has been helpful in getting her to attend school more regularly as well.

Client aware she can use drain the brain practices when having intrusive thoughts or memories to help calm her mind. Also aware to tear up and throw away papers written on after drain the brain practice.

Client/Group progress summary

Client has progressed well. I can tell she feels more in control of her thoughts and emotions. She is notably less anxious and has been attending school more than in the past. Mom also has noted improvements in sleep.

Reflection and self-evaluation

I hope to keep working with this client as she has demonstrated a capacity to apply what is being taught and a motivation to learn more. Energetically, she feels more grounded and I think that strengthening connections with her mother--even though there is an abuse history there--helped her self esteem and sense of safety.

Final Client/Group ReportAfter seeing your client/group (for at least 4 sessions including interactive intake)
Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

1. continue to review breath practices.
2. review how meditative walking practice went.
3. may consider not adding additional practices and having a check in session to see what client wants to do next.
4. continue to include mom, following client's wishes.
