| Session Instructions (Not Mentoring) | Your session outline should be a practice adapted to the needs of that client, including:
Tools from each module should be used (not on each client – but overall) |
| Session Date | 10/01/2024 |
| Session Number | 9 |
| Total Session Minutes | 90 |
| Homework assignment to client/group | • Three-part breathing and meditation 15 minutes daily |
| Activities | The client has a history of lower lumbar disk compression (bulging disk), now generally reduced flexibility, especially of the hips and knees. The session focuses on increasing flexibility and building upper body and core strength. |
| Client/Group progress summary | The client reported that her energy level has improved. She does not feel exhausted after yard work or long hike. The ache and pain are mostly gone. She just got flu and covid shots before the session and her arm was sore. Otherwise, she felt good physically, energetically, and mentally. |
| Reflection and self-evaluation | The client mentioned at the end of the last session that prone position is not comfortable for her because of her large breast. I was glad she finally told me that after 8 sessions. For this session, I avoided prone position completely, although cobra would be okay for her. I discussed with the mentor about how to support the client when she lays on her belly. The client’s arm was sore from vaccinations; I skipped the planned downward dog and plank. |
| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |
| Plan for next session | Continue to work on improving mobility and strength. I will assess overall well-being and fatigue level using PROMIS Global Health v1.2 and PROMIS_SF_v1.0 – Fatigue_10a. |
