Homework assignment to client/group

1. Balancing Exercise (one leg standing, heel-to-toe walking, hip abduction)

2. Lower body muscles strengthen exercises

3. Eye movement: 1 or 2 times daily and as needed.

4. JAPA Meditation with Alternate Nostril Breathing, suggested combining his 10-minute daily practice with the
affirmation, “I trust my body’s ability to heal and restore itself,” which became his favorite mantra.

5. Seated gentle stretch

6. Gentle neck/shoulder stretch

Activities

I began today’s session by asking Dan if he had experienced any new challenges or changes since our last meeting.
He said his daily practice is helping to improve his overall health, including managing stress and dealing with pain. However, he still has balance issues and has a fear of falling. His fall in the shower two years ago may have impacted him emotionally.

I started today’s practice with a gentle breathing guide. Then I led a joint freeing practice on the chair, balancing exercises, and muscle strengthening practices. And he enjoyed and felt a good stretch of lower back by forward bending while holding the back of the chair. Afterward, we moved to the mat to practice spinal twists, sphinx pose, lateral stretches, and child’s pose.
The session ended with 15 minutes of yoga nidra to relax his body and mind, followed by a brief meditation.

Client/Group progress summary

As I observe his progress, I see he has learned tools to manage his existing conditions. These practices help him manage stress, reduce pain, and maintain his motivation.

I learned that sometimes I need to ask further questions when a client addresses their concerns. Dan shared his experience of falling in the shower, which allowed me to connect his balance issues which is physical and fear of falling which is emotional. Otherwise, I wouldn't have considered that the cause of his fear related emotionally

Reflection and self-evaluation

I learned that sometimes I need to ask further questions when a client addresses their concerns.
Dan shared his experience of falling in the shower, which allowed me to connect his physical balance issues with his emotional fear of falling. Otherwise, I wouldn't have considered that the cause of his fear was related to his emotional state.

Final Client/Group ReportAfter seeing your client/group (for at least 4 sessions including interactive intake)
Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

I will continue to follow this care plan for the next several sessions, additional asana will be introduced gradually to strengthen his physical body
