Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Practice Centering and Breathing (Bhramari)

Lying on your back in a quiet, dark space get comfortable using pillows etc. clearing tension in the body, mind and heart, transforming the moment into complete relaxation, then,

a. On an inhalation state a heartfelt pledge to support a life of well-being and meaning

b. With each exhalation imagine that positive energy caressing you from within sheath by sheath, layer by layer. The sheaths:

1. Physical,
2. Energetic (Life Force),
3. Mental & Emotional,
4. Intellectual & Intuitive, and the
5. Bliss sheath.

c. Dwell longer on a layer if you feel the need for extra nurturing.
d. Slowly bring your awareness back to your breath, the body and finally, the external environment to reawaken.

Activities

Anamaya: Chair – centering, Clasped-Hands Head Forward Bend, Back Reverse Arm Hold, Spinal Twist, Backward Bend, One-Legged Forward Fold, Leg Kicks, Leg Raises, Chair Push-ups, Wall Plank.

Pranamaya: Bhramari – How does it feel to block out the senses? What memories are you attached to?

Manomaya: "Melt Tension into Relaxation"; clearing tension in the body, mind and heart, transforming it into complete relaxation and a subtle energy flow.

Vijnjanamaya: Introduce Sutra 2.16 - Prevent future pain from manifesting. A yoga practice is preventive medicine for one’s mind—a way to keep future pain and suffering from manifesting. Using the breath to focus and relieve tension. Talk about the sheaths.

Anadamaya: Allow time for rest and peacefulness; internal and external stillness.

Y & N: Swadhyana – discuss the importance of objectively observing oneself on the inside and out moving through the Koshas

Client/Group progress summary

Client is unable to lift knee/shin/ankle to figure-4. Upper body asanas were welcomed; pain in hip, skipped figure-4, knee to chest, ROM for external rotation is decreasing. Talked about Bhramari and its benefits, - locking out the senses, calming, relaxing, and more. 6 rounds, pause, repeat if comfortable doing so. Sound of the bumble bee, practiced low, mid, and high tones. "The hum of bees is the voice of the garden".

Reflection and self-evaluation

Good discussion on the koshas. Client appeared interested in the sheaths' definitions. In the futures, I'll have a link or two ready to support any new topic. I'll send them to the client. Sessions are flowing better now. Client is comfortable on her chair at the dining room table and on her couch. I wondered if being on her bed would be better, and where she would have more space. I sent some links for setting up a space for her practice. I didn't get the feeling she was interested in doing that. She's in her dining room, their DR and living room have been under construction for months. It would be unsettling for me.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

Check in on her health and physical therapist's recommendations. Review pain level, use motivational interviewing to ask about the patient's intentions, barriers to change, and her strategies to address those barriers. Use guided- imagery to support health and well-being. Bring nature into the imagery based on her input before the meditation.
