Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Homework assignment to client/group

1. Meditation focuses on an uplifting word or Kirtan kriya (sa-ta-na-ma) – 5 min daily
2. Breath – Three-part breathing followed by alternate nostril 5 minutes daily
3. Gentle yoga – 30 minutes once a week.
4. Practice one of Yama or Niyama for 2 weeks

Client/Group progress summary

Two clients did not make it to the class today; one just had foot surgery, and another had a conflict. Breathing patterns and balance have improved.

Reflection and self-evaluation

Because two clients missed the class. The session was only 75 min because one client needed to leave early. Therefore, I have decided to add one more session to the series as I feel that we need a wrap up session when everyone can attend. Everyone is happy to have one more class. I challenged the clients to choose one of Yama and Niyama and practice it for 2 weeks. For accountability, I asked the client to let me know what they have decided to do. Everyone will share their experience at the final class. I will also participate (I have already shared with them about what I will do); hopefully, this will inspire the clients to try it.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

I will continue to include poses that improve flexibility, strength, and balance. The class will share Yama/Niyama practice experience. I will conduct the final balance assessment.
