Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

This report is for the sessions on 9/13,20 and 27
9/13

I asked Reina if there had been any physical or mental changes since we last met.
She reported improvement in her sleep, and meditation can reverse her IBS conditions.
I led a few minutes of center practice to begin. Asana practice began with neck/ shoulder stretch, 3 rounds of sun salutations, chair pose for balancing, 4 restorative poses including
forward bending, Knees to the chest, and supported child pose and lateral stretch.
We conclude the session with 15 minutes of yoga nidra and eye movement.

I asked Reina if there had been any physical or mental changes since we last met.
She reported that she went to aquacise in the morning and felt more energetic after practice.
We started with a brief meditation for a few minutes and eye movement, Then we moved to a series of physical movements, neck/shoulder stretch, sun salutation, tree pose, and then seated poses. These seated poses including long-held supine spinal twit help reduce her LBP and improve her IBS conditions.
To end the session, I led yoga nidra and alternate nostril breathing.

Reina reported that she has been feeling pain in her left glute for the past few days. She thinks it was caused by her walking in regular shoes instead of sneakers. I suggested to don’t overuse and wear shoes when she walks.
I led her guided breathing practice to bring her at the present moment. Today’s physical practice was gentle shoulder stretch, sun salutation, and long-held seated poses (child pose, knees to chest, spinal twist, supine butterfly). To complete the session, I offered 15 minutes of yoga Nidra and a brief meditation.

Client/Group progress summary

As she recovered fully from COVID-19, she showed more improvements.
Her LBP (lower back pain) is gone, her IBS has been significantly improved, and
she sees a dream average once a week now but it's not agitated anymore. She said she would meditate more when she noticed IBS worsened.

Reflection and self-evaluation

Her practice of yoga, meditation, aquacise, and volunteer activity at church are good sources of energy and balance in her daily life. Her husband's health is not good but she can manage herself and support him.
She canceled the last session because she needed to evacuate due to hurricane Milton. I trust her capability of managing herself and supporting others as she has shown progress in the past 3 months.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

I will continue to follow this care plan for the next several sessions. I will introduce new asana or wisdom teaching when I feel it’s appropriate.
